Note that you must build BOTH portals (in the Overworld and in the Wilds) before they can be linked. They must be built according to the rules below, otherwise they cannot be linked.
Each block in the Wilds counts for 12 blocks in the Overworld. Here are formulas for you to reference when figuring out where to build your portals:
In the Overworld: Get the Overworld coordinates of your portal (hit F3 on your keyboard while in-game) and DIVIDE the X and Z coordinates by 12. That will be the location of the portal in the Wilds.
In the Wilds: Inverse of the above - grab the coordinates of your portal in the Wilds and MULTIPLY the X and Z coordinates by 12; this is where your Overworld portal should be.
Height: You can choose at what height your portals will be (aka the Y axis), but note the rules on building materials and portal spacing.
On the Wilds side, portals must be 100 blocks radius away from each other. You will not be able to link a portal from the Overworld to The Wilds if it's too close to another Wilds portal.
Shape: The portal frame can be virtually any shape you'd like, as long as you can fit through it obviously. It can even be horizontal instead of vertical. See photo for example.
Material: The frame must be constructed out of Jungle Log or Jungle Wood blocks.
Contents: There must be some kind of natural/organic material within the portal frame. This can be grass, flowers, ferns, vines, sugarcane, etc. See photo for example.
Caveat: Portals will not function properly if you move through them too fast - this is really only an issue if you're FALLING into a portal.
